The completed form is submitted with the following: a non-refundable registration fee of P500.00 for forms 1-5; an entrance fee of P300.00; a copy of the student's birth certificate and/or national ID (Omang) and/or passport; copies of the parents' national IDs or passports; copies of recent school reports; a transfer letter from the last school attended; and two passport-sized photos.
